Thyroid Problems: The Vague Symptoms People Put Down to Stress

Thyroid Problems: The Vague Symptoms People Put Down to Stress

Thyroid disorders are common, particularly in women, and the symptoms are so ordinary that they get attributed to work, age, having young children, or simply not sleeping enough. The diagnosis is usually a single blood test away.

An underactive thyroid

When the gland produces too little hormone, everything slows: persistent tiredness, weight gain without eating more, feeling cold when others do not, dry skin, hair fall, constipation, heavy or irregular periods, poor concentration and low mood.

An overactive thyroid

Too much hormone speeds things up: weight loss despite a good appetite, a racing or irregular heartbeat, feeling hot and sweating easily, tremor of the hands, anxiety and irritability, difficulty sleeping, and looser stools.

Swelling at the front of the neck

A goitre may be visible or only felt. Bangladesh introduced universal salt iodisation to address iodine deficiency, and it helped considerably, but deficiency has not disappeared everywhere. Any neck swelling — and especially a single firm lump, a hoarse voice, or difficulty swallowing — should be examined.

Thyroid and pregnancy

This deserves particular attention. An untreated underactive thyroid in pregnancy is associated with miscarriage and with effects on the baby’s development, and thyroid problems can also appear in the months after delivery. If you are planning a pregnancy, having difficulty conceiving, or have had recurrent miscarriage, thyroid testing is a reasonable thing to ask about.

Because the symptoms overlap so heavily with everyday life, the test is often what settles the question — not how convincing the story sounds.

The test

A TSH level, usually with a free T4, is enough to answer the question for most people. Further tests are added only if those are abnormal or the picture is unusual.

Treatment

An underactive thyroid is generally treated with a daily tablet, taken on an empty stomach, and the dose is settled by repeating the blood test after a few weeks. It is usually lifelong, and it is usually straightforward. An overactive thyroid has several treatment routes and needs closer supervision. Neither is a reason for alarm, but both are reasons to be tested rather than to keep guessing.
