Bangladesh has one of the most established family planning programmes in the region, and most methods are available free or at low cost through government facilities as well as privately. The difficulty is rarely access; it is choosing well and being able to come back when something does not suit you.
Why spacing matters medically
Leaving at least two years between a birth and the next pregnancy measurably lowers the risk of anaemia, premature birth and low birth weight, and gives the mother’s body time to recover. This is a health question before it is anything else.
Methods you take care of yourself
- The pill — effective when taken daily at roughly the same time. Missed pills are the usual reason it fails, so it suits people with a settled daily routine.
- Condoms — the only method that also protects against sexually transmitted infections. Useful alongside another method.
- Emergency contraception — for after unprotected sex, and more effective the sooner it is taken. It is a backup, not a regular method.
Methods you do not have to remember
- Injectable — one injection every three months. Periods often become irregular or stop altogether, which is not harmful but surprises people who were not warned.
- Implant — a small rod placed under the skin of the upper arm, effective for several years and removable at any time.
- IUD — placed in the womb, effective for several years. The copper type contains no hormones, which suits women who cannot or prefer not to take them.
Long-acting methods are among the most effective precisely because they remove the chance of human error, and fertility returns after removal.
Permanent methods
Sterilisation, for either partner, is appropriate when a couple is certain their family is complete. It should be treated as permanent, and is a decision to make unhurried and together — not during the days after a delivery.
After childbirth and while breastfeeding
Some methods are preferred while breastfeeding, because certain hormones can affect milk supply. Fertility can return before periods do, so relying on breastfeeding alone is unreliable. Discuss this before you leave hospital, or at your postnatal visit.
Most side effects appear in the first three months and then settle. Coming back to change method is far better than stopping quietly and risking an unplanned pregnancy.
Bring your health history
High blood pressure, migraine with aura, smoking over thirty-five, diabetes, liver disease, or a history of clots all influence which methods are safe for you. That is why this is worth a conversation rather than a recommendation from a neighbour or a shop counter.
